Position Assignment Details Department: Home Health ? Physical
Therapy Requirements Graduate of an accredited Physical Therapy
program Active Physical Therapist (PT) license required Current BLS
? Basic Life Support Healthcare Provider certification required
Valid Driver?s License (Class C) required ? Home Health Automobile
Insurance required ? Home Health Minimum 5 years of recent relevant
experience required Skills & Knowledge Strong critical thinking,
complex problem-solving, and independent decision-making skills
Knowledge of professional practice standards and regulatory
requirements Ability to manage multiple priorities in a fast-paced,
demanding work environment Proficiency with computers and required
healthcare technology systems Effective verbal and written
communication skills Strong interpersonal skills including
teamwork, collaboration, conflict resolution, and customer service
Ability to analyze data from multiple sources and identify best
clinical and workflow practices Experience developing treatment
