Job Description:
Coordinate and facilitate care transitions throughout the
hospital stay and across the care continuum Collaborate with
patients, families, physicians, and multidisciplinary teams to
develop and implement care plans Support utilization management by
ensuring appropriate level of care and compliance with protocols
Communicate effectively with third-party payers, review agencies,
claims, and finance departments Assist in discharge planning and
connection to community resources Address complex clinical and
social issues in the Emergency Department, when assigned
